Connect Somerset Champions

Connect Somerset has been launched to improve early help in the community. It is about all professionals and community groups working together to help families and residents to improve their lives. This work also helps to reduce the cost of statutory services.
Connect Somerset is a partnership between Somerset Council, Somerset NHS, Voluntary, Community, Faith and Social Enterprises, and Schools, Colleges and Early Years settings which aims to make it easier for Somerset residents to get the help they need and to get it early.
Champions are there to join up public services and connect to the Voluntary, Community, Faith, Social Enterprise (VCFSE) sector, improving the efficiency of how these services work together.
Champions work collaboratively with public services, ensuring that they are designed to meet the needs of local communities. Champions do not work directly with families and residents.
If you work with residents, working with your local Champion can help you to:
  • Be connected to your community and increase your understanding of the local early help offer delivered by Statutory Services, Education and VCFSE organisations.
  • Improve local processes such as the Team Around the School, Data Sharing, and the Transform database.
  • Be more proactive and increase the volume of early help.
  • Lead a wider strategic view of local needs and a place-based approach to future planning.
View the Connect Somerset Champions map to find your local Champion and how to get in touch.

CQC update on Local authority assessments – published today!

Assessing how local authorities meet their duties under Part 1 of the Care Act (2014) is a new responsibility for CQC.
During the pilots, CQC looked at 9 of their quality statements to assess how well each local authority is meeting its responsibilities. This enabled them to give an indicative rating of the same.

Urgent Community Response (UCR)

Referrals for UCR will be taken via the existing route for all care homes, which is through Somerset Primary Link (SPL) between 8am and 8pm via 01749 836700, option 1.

Urgent community response runs 8am to 8pm 7 days a week.

UCR's aim is to respond to falls within 2 hours, reducing the concerns around long lies and thus reducing pressure to ambulance services if there is no medical need for conveyance to hospital, also providing support when a change to presentation is recognised as a preventative measure.
You can watch a video explaining the Somerset UCR at:
What is the Somerset Urgent Community Response?
Please note that the falls lifting equipment and education has already rolled out to care homes.
You can also find more information on this website for details of services they provide and are not just limited to falls.
